How to refer a patient

Get the CVA Spine referral form (PDF, one page).
Fill it in
Add the patient's information, your practice and contact person, the reason for referral and the patient's insurance.
Fax it with records
Fax the completed form and the records listed below to (559) 599-4536. Please confirm the fax number before sending.
We follow up
Our office will follow up to schedule. Questions about a referral: call (559) 758-7119.
Records to include
- Demographics and insurance cards
- Relevant office notes and treatment history
- MRI, CT and X-ray reports
- Prior spine operative reports, if any
Please note the imaging facility and exam date on the form, and arrange access to the actual images, not just the reports.
Insurance and authorization
List the primary and any secondary plan with member IDs, and mark whether authorization is attached, not required or pending (with the authorization number if you have it). For workers' compensation, include the carrier, claim number and adjuster's name and phone.
Urgent referrals
For an urgent referral, mark "Urgent" on the form and call the clinic at (559) 758-7119 to discuss. For emergencies, use emergency services; please do not rely on fax.
Who we see
Dr. Higginbotham sees patients with cervical, thoracic and lumbar spine problems and adult spinal deformity, for consultations, surgical evaluations and second opinions. See the conditions we treat and the procedures we offer, including endoscopic spine surgery and cervical disc replacement. Patients who already have an MRI can also book a cash-pay virtual MRI review.
